Welcome to Performance Chiropractic & Acupuncture and Signal Mountain Chiropractic (“the Practice,” “we,” “our,” or “us”).

 

By accessing or using our websites www.chattanoogaperformance.com and www.signalmountainchiropractic.com (the “Sites”) and/or receiving any of our services, you agree to be bound by these Terms and Conditions (“Terms”). Please read them carefully before using our Sites or booking services.

1. Services

Performance Chiropractic & Acupuncture and Signal Mountain Chiropractic provide chiropractic care, acupuncture, and related wellness services delivered by licensed professionals. Care plans are tailored to each patient’s individual needs.

Nothing on our Sites should be interpreted as medical advice or a guarantee of specific outcomes. Always consult your primary care provider or other qualified healthcare professionals regarding your health concerns.

 

2. Appointments & Cancellations

  • Appointments can be scheduled through our online booking system, by phone, or in person.

  • We kindly request 24 hours’ notice for cancellations or rescheduling.

  • Appointments canceled with less than 24 hours’ notice, or missed without notice, may be subject to a cancellation fee or forfeiture of the session.

  • Packages and memberships are non-transferable and non-refundable, except as required by law. Unused sessions do not roll over beyond the stated expiration or membership term.

 

3. Payments

  • Payment is due at the time of service unless prior arrangements have been made.

  • We accept major credit and debit cards, cash, and HSA/FSA cards.

  • Package and membership payments are due in full at the time of purchase. Patients can manage package usage and remaining visits through their Jane App patient portal.

  • Prices are subject to change without prior notice. Price changes do not affect appointments or packages already purchased.

 

4. Confidentiality & Privacy

Your privacy matters to us. All personal health information is handled in accordance with HIPAA and other applicable privacy laws.

5. Health & Medical Disclaimer

Our chiropractic and acupuncture services are complementary in nature and are not intended to replace medical care from your physician or other healthcare providers. Individual results may vary depending on your health status and adherence to recommended care plans.

If you have a medical emergency, call 911 or your local emergency services immediately.

 

6. Website Use

By using our Sites, you agree not to:

  • Use the Sites for any unlawful purpose.

  • Attempt to gain unauthorized access to our systems or networks.
